Chapter 5 describes how to comply with American Academy of Pediatrics’ policies on detecting and addressing developmental-behavioral delays and disabilities (including autism spectrum disorders and mental health problems) and how to provide a true medical home. Although AAP policies can appear mind-bogglingly complex and time-consuming, in fact the tasks of surveillance and screening can be accomplished efficiently and within the time-constraints of short preventive service visits. How? By staggering the many tasks across the well-visit schedule.

2. Consent forms you can ask parents to sign so that referral services can share their findings with your practice. The first consent form is a simple agreement to share information (that may need negotiation with referral services). The second consent form was established between practices and one state's IDEA programs in order to comply with both HIPAA and FERPA requirements regarding personally identifying information.
3. A longitudinal problem/process checklist showing when different screening/surveillance tasks need to be performed. This chart is also helpful for documenting your efforts to comply with AAP policy by placing a copy in each child’s chart and updating as you go.
4. Well-visit forms for each encounter that identify which tasks are needed at each age.
